Demand Draft: Demand draft drawn from SBI only for Rs 2000/- in favors of“PAO (HQ), DOT” Payable at Main Branch, NEW DELHI
Enclosures:
1. Two Pass port size Photographs (Name should be written on back)
2. DD
for Rs.2000 for life time license.
3. Gazetted Officer Attested
copy of birth Certificate along with father’s Name.
4. Gazetted Officer Attested
copy of Amateur Result sheet.
