Filling up Ham Radio examination form

Dear Ham Radio applicants!

I am receiving numerous calls from the applicants to explain about various columns in the Ham Radio application form. I am here with uploading complete filled up exam application form along with all the documents. Kindly observe all the filled up application carefully and fill up the application form completely without leaving any columns.

Kindly remember the following things while submitting your Exam application form.

1.       Total 3 pass port size photo’s to be submitted (one photo to be         fixed on Academy of Ham radio application form and the                 remaining 2 photo’s are attached to the main application form.

2.       Conduct & Nationality certificate signed by any gazette officer

3.       Copy of 10^th Class Pass certificate to be attested by any                   Gazetted officer

4.       Copy of Aadhar card to be attested by any any Gazetted officer

5.       Rs 22/ Indian postal stamps or equivalent money for the                   purpose of dispatching resulting sheet

6.       Exam application form should be in the following  sequence

1.  Page1-  Main application

2.  Page 2- Declaration page

3.  Page3-  Following Details may also be furnished                paper

4.  Page4- Following Details may also be furnished paper

5.  Page5- Following Details may also be furnished paper

6.  Page6- Following Details may also be furnished paper

7.  Page7- Conduct & Nationality certificate signed by            any gazette officer

8.  Page8- Copy of 10^th Class Pass certificate to be                  attested  by any Gazetted officer

9.  Page9- Copy of Aadhaar card to be attested by any            Gazetted officer
